Use synthetic, patient-independent examples for this review build. The current no-PHI pilot agreement remains in force until the revised Today policy and organizational reviews are approved.

Start a project

  1. Open Today → Improve. Choose Add editable wait-time and note-in-room examples to try the workflow, or create a project of your own.
  2. Give the project a clear name and, if useful, an aim statement, outcome/process/balancing measures, change ideas, and a first Plan–Do–Study–Act cycle.
  3. Add the fields you will actually collect. A field can be a number, text, dropdown, multiselect, date, or yes/no choice. Mark only essential fields required. Save the project.
  4. Review the plan preview. Use Copy plan or Print / PDF when you need a shareable draft for human review.

For example, a synthetic Patient wait time project can use one required numeric field, Minutes from rooming to clinician entry, with a target of 10 minutes. Do not add names, chart excerpts, visit identifiers, or patient-linked dates to the project or its observations.

Record and review observations

Open the project, enter 8 minutes for a synthetic observation, and choose Save observation. Add more examples to see the count, mean, median, and trend. Use Repeat last to prefill a similar entry, then change its values before saving. You can sort or delete observations and export QI CSV.

Pin a project to make its entry form available on Today. A saved observation remains after Reset for New Visit; an unsaved form does not. Observations are separate QI records and are never linked to the note or a billing encounter.

Connect a personal goal

Open Today → Goals → New goal. Enter a specific target, unit, cadence, action plan, and review date. Choose manual progress, daily encounters, work minutes, or a QI measure as the progress source. For the wait-time example, you might choose at most 10 minutes per week and use the project's numeric measure.

Use Edit / review to add a reflection and Export Markdown for a readable goal summary. Today's optional Suggestions explain the simple data rule behind each prompt, such as a missing first observation or a project mean above your target. Dismiss a suggestion or turn suggestions off in Settings; they are workflow prompts, not clinical judgments.
